SEVEN STEPS TO REGISTERING YOUR CHILD AT DUNWOODY UNITED METHODIST CHURCH PRESCHOOL FOR 2010-2011

1. Complete the Terms and Conditions Signature Page, and Admissions Application.
2. Attach a $100 check per child to the forms and bring to the Preschool on Wednesday, January 20 for church members, Thursday, January 21 for currently enrolled families and Friday, January 22 for families new to our program. Registration will begin at 7:30 a.m. in the gym.
3. If you are registering for 3-day Monday/Wednesday/Friday Twos, or 4-day Tuesday-Friday Threes, you’ll need to attend registration on January 20 (church members) or January 21 (currently enrolled students) at 7:30 am in the gym. Due to limited availability of spots in these classes, everyone must be physically present for the number drawing on their appropriate day. Numbers will be drawn at 7:30 am. There is no advantage to arrive early. If a class fills on the first day, numbers will still be drawn for currently enrolled students to determine a child’s position on the wait list should openings become available.
4. Complete Medical and Emergency Contacts and Health History forms and return these to the Preschool Office by May 1, 2010.
5. Pay September tuition by June 1, 2010 to hold your child’s spot for 2010-2011.
6. Provide a current Georgia Immunization Form 3231 by September 1, 2010.
7. Pay $75 Annual Traffic Fee by September 1, 2010.
